Upon receiving your application, the Administrator will forward an income and asset worksheet so you can provide information necessary to determine, 1) ELIGIBILITY, and, 2) ACCEPTABLE INCOME VERIFICATION for PRE-CERTIFICATION ELIGIBILITY.
